Quality measures are tools that help measure and track the quality of health care services provided by health care professionals, clinics and hospitals within the PQA health care network. These measures use data associated with providers’ ability to deliver high-quality care or relate to long term goals for quality health care.
Tracking and reporting quality measures helps to ensure that our health care system is delivering effective, safe, efficient, patient-centered, equitable, and timely care.

Advance Care Plan
Care PlanPercentage of patients aged 65 years and older who have an advance care plan or surrogate decision maker documented in the medical record or documentation in the medical record that an advance care plan was discussed but the patient did not wish or was not able to name a surrogate decision maker or provide an advance care plan.
